In the event the credit application
is approved, the applicant hereby agrees to and accepts the following
terms and conditions: Full payment shall be due upon receipt of
statement. Failure to make payment in full within thirty days of
statement closing date will subject applicant’s account to a Finance
Charge, which will be computed on the average daily balance at a monthly
rate of 1% (Annual Percentage Rate of 12%)
If payment is in arrears in excess of ninety days, Boston Corporate
Transportation Inc. shall have the right to charge against any credit
card used as back up security of the account.
In the event that the account remains unpaid and legal fees and costs
are incurred by Boston Corporate Transportation Inc. relating to the
collection thereof, the applicant agrees that it shall be liable for any
and all such reasonable legal fees and costs, in addition to its
outstanding balance.
The applicant hereby agrees that the usual credit inquiries may be made
and it authorizes Boston Corporate Transportation Inc. to obtain such
information as it may require from whatever sources it deems necessary
concerning any statements made on the credit application. The applicant
certifies that the credit application statements are true, correct and
complete and have been made for the purpose of inducing Boston Corporate
Transportation Inc. to establish credit for the applicant. It is the
information included on this application Boston Corporate Transportation
Inc. will rely thereupon.
The applicant represents that he/she is an officer and/or is duly
authorized to file this application for Boston Corporate Transportation Inc.
credit charge.
Boston Corporate Transportation Inc. is not responsible for personal
property left in the vehicles.

| NO SHOW AND CANCELLATION POLICY |
In Los Angeles, New York City, San
Francisco and Washington, DC we require a minimum of two hours notice
prior to the pick-up time in order to waive our minimum charge for autos
and vans. Buses require a twenty-four hour notice. On the day of
extraordinary events (including, but not limited to the Oscars, Grammys,
New Years Eve, Emmys, Latin Grammys, Soul Train Music Awards, Academy of
Country Music Awards, MTV VMA, AMA and the Rose Bowl) extended minimums
apply which require as much as 24-hours notice to cancel without
penalty. For the rest of the continental United States and Canada a two
hour notice might be sufficient, but might require as much as
twenty-four hours depending on the market. A cancellation notice without
fee of twenty-four hours is required for Central and South America and
Europe. Forty-eight hours for the Far East and Australia. Late
cancellations and no shows are charged at the respective minimums for
that market and vehicle type. If the reservation is canceled within the
specified time allotments, it is the client’s responsibility to obtain
and note a cancellation number as given to you by Boston Corporate
Transportation and will be responsible for any charges incurred by not
giving proper notification. Applicant agrees to Boston Corporate
Transportation terms, conditions and rates.
